Medical.-.Fitness For Duties Assessment Business rule last reviewed and updated by S32 August 2021 | Fitness for duties requirement: · Letter must include Contracting Company’s logo and Contracting Company’s supervisor/manager/HSE representative’s signature. · Letter must be outlined as below: EMPLOYEE is employed as a JOB TITLE at COMPANY and has been in the position for NUMBER years. In total EMPLOYEE has NUMBER years’ experience as a JOB TITLE. EMPLOYEE has been medically assessed and is currently fit to perform the work associated with this role for Illawarra Metallurgical Coal. COMPANY will inform the Illawarra Metallurgical Coal representative of any change to EMPLOYEE’s fitness to undertake this work. From 1 Feb 2020, fitness for duties competency will require a pre-employment medical to demonstrate the contractor is able to perform the inherent requirements of the role and meet the Appendix A – minimum medical requirements (IMCP0185) within the Medical Assessment Procedure. A functional capacity evaluation report will also need to be included under the fitness for duties competency. Exemption Note: Ad-hoc contractors who complete limited work annually may continue to provide a Fitness for Duties letter following South32 Department Manager’s approval.
