Fitness for duties requirement: · Letter should include company must include Contracting Company’s logo and Contracting Company’s supervisor/manager/HSE representative’s signature. · Letter should must be outlined as below: EMPLOYEE is is employed as a a JOB TITLE at at COMPANY and and has been in the position for for NUMBER years. In total total EMPLOYEE has has NUMBER years’ years’ experience as a a JOB TITLE. EMPLOYEE EMPLOYEE has been medically assessed and is currently fit to perform the work associated with this role for Illawarra Metallurgical Coal. COMPANY will will inform the Illawarra Metallurgical Coal representative of any change to to EMPLOYEE’s fitness fitness to undertake this work. From 1 Feb 2020, fitness for duties competency will require a site access medical completed by Coal Service or a pre-employment medical to demonstrate they are the contractor is able to perform the inherent requirements of the role and meet the the Appendix A – minimum medical requirements (IMCP0185) within the the Medical Assessment Procedure. A functional capacity evaluation report will also need to be included under the fitness for duties competency. Exemption Note: Ad-hoc contractors who complete limited work annually may continue to provide a Fitness for Duties letter following South32 Department Manager’s approval.
