Operator.Training.Aerial Lift Operator Training

(iii) Training of employees in the operation and inspection of working platforms shall be done by a competent person. (iv) Written work procedures for the operation, safe use and inspection of working platforms shall be provided for employee training. Pictorial methods of instruction may be used, in lieu of written work procedures, if employee communication is improved using this method. The operating manuals supplied by manufacturers for platform system components can serve as the basis for these procedures.

(v) The employer shall certify that employees have been trained in operating and inspecting a working platform by preparing a certification record which includes the identity of the person trained, the signature of the employer or the person who conducted the training and the date that training was completed. The certification record shall be prepared at the completion of the training required in paragraph (i)(1)(ii) of this section, and shall be maintained in a file for the duration of the employee’s employment. The certification record shall be kept readily available for review by the Assistant Secretary of Labor or the Assistant Secretary’s representative.\

Technician Conveyor.-.Conveyor Safety

(a)The employer must provide training to each employee whose work is regulated by this standard, at no cost to the employee, and ensure that the employee possesses the understanding, knowledge, and skills necessary for the safe performance of the duties assigned under this standard. This training must result in an understanding of the hazards in the permit space and the methods used to isolate, control or in other ways protect employees from these hazards, and for those employees not authorized to perform entry rescues, in the dangers of attempting such rescues. (b) Training required by this section must be provided to each affected employee: (1) In both a language and vocabulary that the employee can understand; (2) Before the employee is first assigned duties under this standard; (3) Before there is a change in assigned duties; (4) Whenever there is a change in permit space entry operations that presents a hazard about which an employee has not previously been trained; and (5) Whenever there is any evidence of a deviation from the permit space entry procedures required by paragraph 1926.1204(c) of this standard or there are inadequacies in the employee’s knowledge or use of these procedures. (c) The training must establish employee proficiency in the duties required by this standard and must introduce new or revised procedures, as necessary, for compliance with this standard.

Operator Truck.Qualified.Powered Industrial Truck (PIT)/Forklift Operator

(l) - Operator training. (1) - Safe operation. (i) The employer shall ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l). (ii) Prior to permitting an employee to operate a powered industrial truck (except for training purposes), the employer shall ensure that each operator has successfully completed the training required by this paragraph (l), except as permitted by paragraph (l)(5). (2) - Training program implementation. (i) Trainees may operate a powered industrial truck only: (A) - Under the direct supervision of persons who have the knowledge, training, and experience to train operators and evaluate their competence; and (B) - Where such operation does not endanger the trainee or other employees. (ii) Training shall consist of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace. (iii) All operator training and evaluation shall be conducted by persons who have the knowledge, training, and experience to train powered industrial truck operators and evaluate their competence. (3) - Training program content. Powered industrial truck operators shall receive initial training in the following topics, except in topics which the employer can demonstrate are not applicable to safe operation of the truck in the employer's workplace. (i) Truck-related topics: (A) - Operating instructions, warnings, and precautions for the types of truck the operator will be authorized to operate; (B) - Differences between the truck and the automobile; (C) - Truck controls and instrumentation: where they are located, what they do, and how they work; (D) - Engine or motor operation; (E) - Steering and maneuvering; (F) - Visibility (including restrictions due to loading); (G) - Fork and attachment adaptation, operation, and use limitations; (H) - Vehicle capacity; (I) - Vehicle stability; (J) - Any vehicle inspection and maintenance that the operator will be required to perform; (K) - Refueling and/or charging and recharging of batteries; (L) - Operating limitations; (M) - Any other operating instructions, warnings, or precautions listed in the operator's manual for the types of vehicle that the employee is being trained to operate. (ii) Workplace-related topics: (A) - Surface conditions where the vehicle will be operated; (B) - Composition of loads to be carried and load stability; (C) - Load manipulation, stacking, and unstacking; (D) - Pedestrian traffic in areas where the vehicle will be operated; (E) - Narrow aisles and other restricted places where the vehicle will be operated; (F) - Hazardous (classified) locations where the vehicle will be operated; (G) - Ramps and other sloped surfaces that could affect the vehicle's stability; (H) - Closed environments and other areas where insufficient ventilation or poor vehicle maintenance could cause a buildup of carbon monoxide or diesel exhaust; (I) - Other unique or potentially hazardous environmental conditions in the workplace that could affect safe operation. (iii) The requirements of this section. (4) - Refresher training and evaluation. (i) Refresher training, including an evaluation of the effectiveness of that training, shall be conducted as required by paragraph (l)(4)(ii) to ensure that the operator has the knowledge and skills needed to operate the powered industrial truck safely. (ii) Refresher training in relevant topics shall be provided to the operator

Rigger.Certificate.Rigger Certification

NCCCO - National Commission for the Certification of Crane Operators
NCCER - National Center for Construction Education & Research

NCCCO offers level 1 and 2 Certified Rigging tests that are conducted by a proctor. The written exam consists of 60 questions which have to be completed in 60 minutes. The practical for the certified rigging exam demonstrates basic rigging competency.

Rigger.Qualified.Rigger Qualification

§1926.1401 Definitions
Qualified person means a person who, by possession of a recognized degree, certificate, or professional standing,
or who by extensive knowledge, training and experience, successfully demonstrated the ability to solve/ resolve
problems relating to the subject matter, the work, or the project.

Qualified rigger is a rigger who meets the criteria for a qualified person.
 
From § 1926.1404 Assembly/Disassembly—general requirements (applies to all assembly and disassembly operations) (r)
Rigging. In addition to following the requirements in 29 CFR 1926.251 and other requirements in this and other
standards applicable to rigging, when rigging is used for assembly/ disassembly, the employer must ensure that:
(1) The rigging work is done by a qualified rigger.
(2) Synthetic slings are protected from: Abrasive, sharp or acute edges, and configurations that could cause a reduction of the sling’s rated capacity, such as distortion or localized compression.
Note: Requirements for the protection of wire rope slings are contained in 29 CFR 1926.251(c)(9).
(3) When synthetic slings are used, the synthetic sling manufacturer’s instructions, limitations, specifications and recommendations must be followed.

From § 1926.1425 Keeping clear of the load (c) When employees are engaged in hooking, unhooking, or guiding the load, or in the initial connection of a load to a component or structure and are within the fall zone, all of the following criteria must be met:
(1) The materials being hoisted must be rigged to prevent unintentional displacement.
(2) Hooks with self-closing latches or their equivalent must be used. Exception: ‘‘J’’ hooks are permitted to be used for setting wooden trusses.
(3) The materials must be rigged by a qualified rigger.

Radiation Maintenance.Certificate.Radiation, Ionizing

(f) Immediate evacuation warning signal. (viii) All employees whose work may necessitate their presence in an area covered by the signal shall be made familiar with the actual sound of the signal-preferably as it sounds at their work location. Before placing the system into operation, all employees normally working in the area shall be made acquainted with the signal by actual demonstration at their work locations. (i) Instruction of personnel, posting. (2) - All individuals working in or frequenting any portion of a radiation area shall be informed of the occurrence of radioactive materials or of radiation in such portions of the radiation area; shall be instructed in the safety problems associated with exposure to such materials or radiation and in precautions or devices to minimize exposure; shall be instructed in the applicable provisions of this section for the protection of employees from exposure to radiation or radioactive materials; and shall be advised of reports of radiation exposure which employees may request pursuant to the regulations in this section. (3) Each employer to whom this section applies shall post a current copy of its provisions and a copy of the operating procedures applicable to the work conspicuously in such locations as to insure that employees working in or frequenting radiation areas will observe these documents on the way to and from their place of employment, or shall keep such documents available for examination of employees upon request.

Operator Excavator.Qualified.Trenching & Excavation Competent/Qualified Person

OSHA Reference: Construction 1926.32(f)
"Competent person" means one who is capable of identifying existing and predictable hazards in the surroundings or working conditions which are unsanitary, hazardous, or dangerous to employees, and who has authorization to take prompt corrective measures to eliminate them.

Certain activities or safety procedures at a construction site require design, inspection or supervision by a competent person. The OSHA Construction Standard defines a competent person as someone who is:

  • capable of identifying existing and predictable hazards in the surroundings, or

  • working conditions which are unsanitary, hazardous, or dangerous to employees, and

  • who has authorization to take prompt corrective measures to eliminate them.

Trenching and excavation work is dependent on these specialized employees because its highly technical nature, as well as its inherent hazards, require a greater level of training and experience than a normal worker would possess. The items below specify the trenching and excavation activities where a competent person is necessary.
Protective Systems or Equipment

  • Monitoring water removal equipment and operations. [29 CFR 1926.651(h)(2)]

  • Inspecting excavations subject to runoff from heavy rains to determine need for diversion ditches, dikes, or other suitable protection. [29 CFR 1926.651(h)(3)]

  • Determining cave-in potential to assess need for shoring or other protective system. [29 CFR 1926.652(a)(1)]

  • Examining damaged material or equipment used for protective systems to determine its suitability for continued use. [29 CFR 1926.652(d)(3)]

  • Classifying soil and rock deposits, by both visual analysis and by testing, to determine appropriate protection; re-classifying, if necessary, based on changing conditions. [29 CFR 1926 Subpart P Appendix A]

  • Determining the appropriate slope of an excavation to prevent collapse due to surcharge loads from stored material or equipment, operating equipment, adjacent structures, or traffic, and assuring that such slope is achieved. [29 CFR 1926 Subpart P Appendix B (c)(3)(iii)]

Inspecting Trench and Protective Systems

  • Authorizing immediate removal of employees from the hazardous area where evidence of possible cave-in, failure of protective systems, hazardous atmospheres, or other hazardous conditions exists. [29 CFR 1926.651(k)(2)]

Unsafe Access/Egress

  • Designing structural ramps that are used solely by employees as a means of access or egress. Structural ramps used for access or egress of equipment must be designed by a competent person qualified in structural design. [29 CFR 1926.651(c)(1)(i)]

OSHA Reference: General Industry 29 CFR 1910.119

(c) A blaster shall be qualified, by reason of training, knowledge, or experience, in the field of transporting, storing, handling, and use of explosives, and have a working knowledge of State and local laws and regulations which pertain to explosives.

Acceptable training documentation should include the following:

OSHA Reference: Construction 29 CFR 1926.650 – 652 Subpart

All employers, supervisors and workers whose job duties require them to be in or around trenching and excavation work should have knowledge of concepts such as soil types, soil testing, types of trench collapses, trench protection, and safe work practices.

Acceptable training documentation should include the following:
