• Name on Licence  / card / Statement of Attainment to match the person registered
• Relevant Class / Code to be listed or show indication of a Statement of Attainment unit of competency with corresponding coding (ie LE, LL, LG etc)
• Black and white or colour copy acceptable
• Needs to be issued by a training company with ABN but does NOT need to be issued by Registered Training Organisation (RTO)

• Issue Date = As per Document

• For the Statement of Attainment or Transcript:
  • Applicants name to be shown on document
  • Applicants name on document must match cardholders registered name (However shortened versions of first names such as 'Chris' for 'Christopher' is acceptable
    
    Note: Cards are accepted in lieu of statement of attainment certificates providing they show the relevant statement of attainment unit of competency as well as all other necessary information relating to applicant details and RTO details
    
    Note: Temporary / Interim statement of attainment evidence or letters indicating they are for temporary issue until statement of attainment certificate is provided to applicant is acceptable for a period of 4 weeks only from the date of the training being complete which should be indicated on the letter providing it has been issued by an RTO and indicates completion of a nationally recognised unit code
    
    
• Must include:

  • RTO Name, Code and Logo

  • Units of competency code and name, date completed

    • Pegasus Administrators to click HERE for additional information on checking superseded unit of competency codes

  • Authorised person name or title (does not necessarily need a signature)

  • Issuing organisation’s logo / corporate identifier / unique watermark and / or document or certificate number

  • Nationally Recognised Training (NRT) Logo (For a Statement of Attainment only - not card)

  • Document verifier to ensure the Issue of this document is an RTO

• Issue Date = As per supplied evidence
• Expiry Date =
  • For Statement of Attainment / Transcript / Cards = No expiry
  • For Temporary / Interim Statement of Attainment evidence or letters = 4 week expiry

These competencies are non-accredited training/instruction and are not a nationally recognised qualification, statement of attainment, or licence.

Acceptable forms of evidence include:

1. Internal certificate of completion
2. Certificate of attendance
3. Letter of confirmation from an external provider or written by the company

• Must indicate completion of matching competency title

• Statement of Attainment must display "RIIHAN309F - Conduct telescopic materials handler operations"
• Statement of Attainment must be issued by a registered training organisation (RTO)
• No expiry date is required for this competency upload

Alternate Evidence in the form of "TSH Trained Operator Gold Card":

• Must show persons first and last name
• Must have photo
• Must state Gold Card
• Must be issued by TSHA
• Must have unique card number

• Issue Date = Onsite should be recorded as the date of completion shown on the Statement of Attainment or date of training as stated on the Gold Card
• Expiry Date = Not required for Statement of Attainment however for the Gold Card as stated on the card
