All 6 pages of the form must be uploaded Page 1 - Name to match registering person
- Position must be completed
- Contractor Company must be completed
- Must select Primary WHC site
- Induction(s) required – must be completed
- Approval section: must be signed EXCEPT contractor owner or coordinator field (No longer Required)
Additional Requirements - For Tradesperson roles (any roles including ‘Tradesperson’ in the name): Must include sign off by a person on the authorised approvers list relevant to the role and site. Eg. Electrical Tradesperson to be signed by Manager Electrical Engineering, all other Tradesperson roles to be signed off by Mechanical Engineering Manager
- All Open Cut – Operator must be signed off by the Manager of Mining Engineering (MME) or Fixed Plant and CHPP Manager
- Narrabri - The Manager of Mining Engineering (MME) needs to sign for ALL underground trades and Operator roles.
- Narrabri - MME sign off not needed for surface role at Narrabri OR for re-inductees

Page 3 - Roles - Where Narrabri is selected as the primary site on page 1 a Narrabri role must be selected.
Page 4, 5 & 6 Form must be no older than 3 months - refer to the latest sign-off date in the approvals section.
