Medical.-.NSW Coal Compliant Medical (Order 43) Rule for order 41 medicals | - Name on medical to match the person registered
- FULL medical to be uploaded
- Part D (Traffic light page) certificate of fitness must be accompanied by the Order 41 compliance confirmation email from CS Health and must say “Order 41 Compliance status – compliant”.
- The Preplacement medical detailed certificate completed by CS Health is accepted and does not require the confirmation of compliance to be attached.
- Doctors signature, signed date and stamp mandatory
- Section 4 Queensland Coal Board medicals are NOT accepted
- If AMBER, Pegasus escalate to Thiess
- Medicals issued on or after 1 July 2018 MUST be an Order 43
- Medicals issued before 1 July 2018 that are still within the 3 year expiry period CAN be an Order 41
For Internal THIESS Employees Only: For medicals and chest xrays for Thiess workers only, a letter (company letterhead) with medical date and chest xray date will be accepted instead of the actual medical and chest xray documents | - Issue Date = Examination date
- Expiry Date = 3 years from date of examination (unless review date listed in comments)

- Name on medical to match the person registered
- FULL medical to be uploaded
- Date of birth to match person registered
- Date of assessment listed
- Employer must be listed – Employer must be the current employer. If the employee has changed companies than the current employer must submit the medical to CS Heath and provide a confirmation of registration with the medical.
- Must have a SEG number
- Follow up section – if a review time is indicated, this must match the date recommended in the recommendation/restrictions section
- Must be completed by an approved medical practitioner on the list below

Periodic Medical Assessment – External medical provider – CS Services - Blue cover page
Must have Doctors signature, must have name and date. Site Access Plus Periodic Medical Assessment Detailed Certificate - CS Health
- All Pages required to be uploaded – site access plus periodic medical assessment detailed certificate.
- Name on site access plus periodic medical assessment detailed certificate to match the person registered
- Date of birth on assessment certificate ad assessment report to match the person registered
- Must have a SEG number on the assessment report
- Follow up section – if a review time is indicated, this must match the date recommended in the recommendation/restrictions section on the assessment certificate
- Must be completed by an approved medical practitioner on the list below

Medical.-.Chest X-Ray - ILO Report | For Contractors:
- Applicant Name on ILO report to match the person registered
- Applicant DOB to be displayed on report and to match DOB that’s recorded
- X-Rays: The full ILO chest X-Ray report must be provided. If the X-Ray report suggests a follow up, HRCT is required and this must also be uploaded
- The X-Ray must be read to ILO standards and current (within the order 43 requirements - currently 3 years)
- Must indicate that it is a Chest Xray report (ILO classification)
- ILO report result to indicate no abnormalities report (ILO classification)
- Must be dated
- Must be confirmed by the Doctor by either way of Doctors signature, Verified by or Authorised by (Electronic Signatures are Accepted)
OR - If an ILO report is not available: reload a copy of the FULL Order 43 Medical that clearly displays the Applicants name, Applicants DOB and that the X-Ray outcome was normal or still current. For Order 41 - Certificate of fitness to be uploaded - ensure 'Date of last chest x-ray' section is completed and x-ray is still current, that clearly displays the Applicants name, Applicants DOB.

- Expiry Date = 3 years from date of examination (unless a review date is listed then enter the expiry as the earliest expiring date)

Drug and Alcohol Assessment | - Name and DOB on medical to match the person registered
- Drug and alcohol test completed by an NATA accredited provider
- MUST have been completed within 30 days of the role application
- 0 BAC and All Negative for Drugs.

- D & A screening must be in accordance with AS4308 Urine testing shall screen for:
- - Amphetamines
- - Benzodiazepines
- - Opiate
- - Cannabinoids metabolites
- - Cocaine Metabolites
