Medical.-.COVID-19 Certificate

• Acceptable evidence can be in the form of:

  • COVID-19 Vaccination Record Card issued by the CDC

• Evidence must state that the applicant has had at least one dose

• Applicant's name should be displayed on the evidence and should match the name registered in the system (however shortened versions of names such as 'Chris' for 'Christopher' is acceptable)

• Date of Birth to be listed and match the DOB registered in the system

• Colour copy or black and white copy is acceptable

If one dose administered:

• Issue Date = Date of first dose

• Expiry Date = 3 months

If two doses administered:

• Issue Date = Date of second dose

• Expiry Date = None

Medical.-.COVID-19 Exemption

• Acceptable evidence can be in the form of:

  • A letter from a doctor

• Applicant's name should be displayed on the evidence and should match the name registered in the system (however shortened versions of names such as 'Chris' for 'Christopher' is acceptable)

• Date of Birth to be listed and match the DOB registered in the system

• Colour copy or black and white copy is acceptable

• Issue Date = Date on letter

• Expiry Date = None

Safety.Course.Respiratory Fit Test

• Acceptable evidence can be in the form of:

  • Respiratory Fit Test Report

• Evidence must indicate that the applicant has passed the fit test

• Applicant's name should be displayed on the evidence and should match the name registered in the system (however shortened versions of names such as 'Chris' for 'Christopher' is acceptable)

• Colour copy or black and white copy is acceptable 

• Issue Date = Date of completion

• Expiry Date = 1 year
