Health & Safety.Course.Contractor Safety Passport

The required evidence will be a:

  • City and Guild, NVQ, certificate or competency card showing qualified test & inspection of Plant Machinery

Course completion date must be visible
Applicant’s name should be displayed on the evidence and should match the applicant’s name that is registered in Pegasus (However shortened versions of names such as 'Chris' for 'Christopher' is accepted).
Copy must be clear and legible
Can be in colour copy OR black and white copy

  • Issue Date = Date of completion

IOSH Managing Safely

(Institution of Occupational Safety and Health)

Certificate to be issued by IOSH with:

  • The applicants name on the document must match the applicants name as it has been entered into Workforce Management.

  • Photo to match the person

  • Date of Birth (DOB) to match applicants DOB in Onsite (if there is one displayed on the evidence)

  • Must be current

  • Black and White OR Colour copy is acceptable

  • Scanned document to be clearly legible

  • Issue Date = Date of completion

  • Expiry Date = Not Required
