Safety.Qualified.Respiratory Fit Test

• Acceptable evidence can be in the form of:

  • Respiratory Fit Test Report

• Evidence must indicate that the applicant has passed the fit test

• Applicant's name should be displayed on the evidence and should match the name registered in the system (however shortened versions of names such as 'Chris' for 'Christopher' is acceptable)

• Colour copy or black and white copy is acceptable 

• Issue Date = Date of completion

• Expiry Date = 1 year
