Health.-.COVID Vaccine Exemption | Acceptable forms of evidence: - COVID-19 Vaccine Medical Contraindication Form
As per the NSW government website: If, as an authorised construction worker from a local government area of concern, you have a medical reason as to why you cannot receive a vaccination, you need to obtain a medical contraindication certificate from a medical practitioner (such as your doctor) who must use a form approved by the NSW Chief Health Officer. - Form must have NSW government logo
