  • Company name and ABN must be listed on the form and must match the company name that is registering the plant in the system 
  • Evidence to list asset details
  • Evidence must relate to assets in all areas
  • Must be completed within the last 12 months prior to being on site
  • Issue date to be listed as per the form 
  • Expiry date to be listed as per document 

Lift / Tie Down Certification
  • Company name and ABN must be listed on the form and must match the company name that is registering the plant in the system 
  • Evidence must state details of asset
  • Evidence must have expiry date 
  • Issue date to be listed as per the form 
  • Expiry date to be listed as per document 

List of Operators 
  • Company name and ABN must be listed on the form and must match the company name that is registering the plant in the system 
  • Evidence must state asset details
  • Evidence must state list of operators including Onsite ID number 
  • This must be updated/renewed in the system when a new operator is added to the list 
  • Issue date to be listed as per the form 

Load Charts
  • Company name and ABN must be listed on the form and must match the company name that is registering the plant in the system 
  • Evidence must state asset details
  • Issue date to be listed as per the form 

Maintenance Report
  • Company name and ABN must be listed on the form and must match the company name that is registering the plant in the system 
  • Evidence must state asset details
  • All sections of checklist must be filled in
  • Must be completed within 12 months of coming to site
  • Expiry date - 6 months
  • Issue date to be listed as per the form
  • Expiry listed 6 months from issue date

Plant Inspection Checklist
  • Company name and ABN must be listed on the form and must match the company name that is registering the plant in the system 
  • Evidence must state asset details
  • All sections of checklist must be filled in
  • In asset leaves site, this is to be filled in upon returning and resubmitted in the system
  • Issue date to be listed as per the form 

Risk Assessment
  • Company name and ABN must be listed on the form and must match the company name that is registering the plant in the system 
  • Evidence to list asset details
  • Evidence must relate to assets in all areas
  • Must be completed within the last 12 months prior to being on site
  • Issue date to be listed as per the form 
  • Expiry date to be listed as per document

Annual Inspection
  • Company name and ABN must be listed on the form and must match the comapny name that is registering the plant in the system 
  • All components of the form must be filled in
  • Expiry is 12 months
  • Issue date to be listed as per the form 
  • Expiry date to be listed as per document or 12 months from issue date

  • Image must be taken of the plant being registered in the system
  • Image must be clear and include the entire compliance plate
  • All details must be visible on the plate
  • Issue date to be recorded as per compliance plate

General Condition Photo
  • Image must be taken of the plant being registered in the system
  • Image must be clear and include the entire asset from either a front or side view angle
  • Asset number must be included in the photo 
  • Issue date to be recorded as date of asset registration in system
