Food Management.Qualified.Do Food Safely / GMP | - Workers can either:
- Complete the GMP HFC delivered Do Food Safely Online Training
or - Upload a copy of their FBPOPR3021 Apply good manufacturing practice requirements in food processing statement of attainment
or - Upload a copy of their Do Food Safely completion certificate as issued by the Victoria Department of Health

- Business Rules for the Do Food Safely completion certificate are:
- Applicants name to be displayed on certificate and to match applicants registered name (however shortened versions of names such as 'Chris' for 'Christopher' is accepted)
- Certificate must be issued by the Victorian Department of Health
- Black and white or colour is acceptable
| - Issue Date = as shown on statement of attainment or certificate
- Expiry Date = not required

Food Management.Certificate.Food Handler - Vic (both SITXFSA005 & SITXFSA006) | - Applicants name to be displayed on certificate and to match applicants registered name (however shortened versions of names such as 'Chris' for 'Christopher' is accepted)
- Certificates must state both SITXFSA005 & SITXFS006
- Can be uploaded as a single certificate displaying both course codes, or two separate certificates
- Must be issued within the last 5 years
- Black and white or colour is acceptable
| - Issue Date = as shown on certificate
- Expiry Date = 5 years from issue date
Note: If an existing document expires on /is expiring after 08 Dec 2023 expiration date is to be entered as 08 Dec 2028. If issue date falls after 08 Dec 2023, please enter the true to date expiration. |

- Certificate must state Food Safety Supervisor
- Must be issued within the last 5 years
- Black and white or colour is acceptable
| - Issue Date = as shown on certificate
- Expiry Date = 5 years from issue date
