Welder.Certificate.Welding Supervisor

  • Applicants name on the document must match name in the system (however shortened versions such as 'Chris' for 'Christopher' are acceptable)
  • Evidence will be a certificate showing completion of 'Welding Supervisor' course referencing standards AS 2214-2004
  • Evidence will be issued by Welding Qualification and Certification Boards such as WTIA : Welding Technology Institute of Australia or Weld Australia
  • Issue Date = Date of completion
  • Expiry Date = No expiry date to be entered

Welder.-.WTIA Licence - AS2214 Welding Supervisor
  • Applicants name on the document must match name in the system (however shortened versions such as 'Chris' for 'Christopher' are acceptable)
  • Evidence will be a licence showing completion of 'Welding Supervisor' course referencing standards AS 2214-2004
  • Evidence will be issued by Welding Qualification and Certification Boards such as WTIA : Welding Technology Institute of Australia or Weld Australia
  • Issue Date = Date of completion
  • Expiry Date = No expiry date to be entered

Welder.Course.Certified Welding Inspector

  • Applicants name on the document must match name in the system (however shortened versions such as 'Chris' for 'Christopher' are acceptable)
  • Evidence will be a certificate or card showing completion of 'Welding Inspector' or 'Welding Supervisor'
  • Evidence will be issued by Welding Qualification and Certification Boards such as WTIA : Welding Technology Institute of Australia or Weld Australia

Welder.Trade.Qualification - Welding (First Class)

Site.-.NDT; AINDT or PCN Accreditation Level 1
  • Applicants name on the document must match name in the system (however shortened versions such as 'Chris' for 'Christopher' are acceptable)
  • Evidence will show recognition of Non Destructive Testing (also referred to as ISO 9712) with 'Level 1' being explicitly nominated.
  • Evidence must be issued by either AINDT or PCN
  • Issue Date = As shown on the document
  • Expiry Date = As shown on the document

Site.-.NDT; AINDT or PCN Accreditation Level 2
  • Applicants name on the document must match name in the system (however shortened versions such as 'Chris' for 'Christopher' are acceptable)
  • Evidence will show recognition of Non Destructive Testing (also referred to as ISO 9712) with 'Level 2' being explicitly nominated.

      Matching competency name and code (if with checkbox, this should be completed)
      Applicant's signature and signed date
      Assessor's complete name, signature, and date
      Must show that the person is competent and authorised to operate

NOTE:

  • If Site Authorisation, the RCC Assessment or CTO Assessment section must be completed

  • If Theory Assessment, when RCC is completed, the authorised leader is required to verify with a signature that the applicant is authorised to operate

  • The learning and development section of the form does not need to be completed

Issue date = If a certificate has been provided, use the course completion date as issue date. If Theory Assessment or Site Authorisation, use the assessor's signature date.

Expiry date = 99 years from issue date
