Proof of Identity

  • Must be an Australian Government issued document (birth certificate, 18 plus card, licence, passport)
  • Front & back of licence or 18+ card to be uploaded
  • Name on the document to match the person registered
  • Must be legible
  • International Licence accepted
  • Issue date (if there is one shown on the document) is required to be entered in Onsite.
    • Expiry date required to be entered into Onsite if there is an expiry date recorded as there is on passports / licences and photo cards


    • Name on the document to match the person

    Acceptable forms of evidence include: 18+ card, licence, passport, International drivers licence and International passport


    Digital Drivers Licence 

    When submitting a digital drivers licence from the Service NSW app screenshots of the full licence must be submitted to ensure that all information (licence, card number and address information) is provided - Note that 2x screenshots will likely need to be taken in order to capture all of this information.


    Passport

    Must show page with applicants name, photo, DOB etc. The worker name on ID must match the name in the system. Must be current / valid.


    Interim or Temporary licences

    Interim or temporary licences can be accepted for a period of 30 days, after which a photo ID must be provided
