"Herrera" instead of "Herrera Trujillo"
"Mohd" instead of "Raza Hasan Mohd"

The following evidence can be accepted:
  • SAP Extract
  • If MSIC Card, it must issued by an authorised issuing body on behalf of Australian Government

Matching competency name, with or without code (if with checkbox, this should be completed)

Issue date = as shown on the evidence. Can be the date of Induction or Attendance

Expiry date = 2 years from issue date

NOTE: The card will only show a month and year. Use the last date of the month as the expiry date.

Example: Aug 2022 on card. Input 31-Aug-2022 as the expiry date.
