- Evidence will be ADAS Licence showing 'Dive Supervisor' as the achieved qualification
- Must be Issued by Australian Diver Accreditation
- Both sides of the licence must be displayed within uploaded file
- Black and white or colour is acceptable
- Scanned document to be clearly legible
