• Operator License or Qualification (Depends on Equipment)

• Applicants name to be shown on document and must match cardholders registered name in WFM (however shortened versions of first names such as 'Chris' for 'Christopher' is acceptable)

• A copy of the Cargill Statement of competence form must be downloaded from this page, completed and then uploaded into the workforce system

• All sections to be completed

• The acknowledgement section must be completely filled out and signed by a manager or supervisor

• Issue Date = Date of completion

• Expiry Date = 3 years
