  • Applicants name to be shown on documentation
  • Applicants name on document must match name that has been entered into Onsite
  • Document must show completion of the Working With Children Check
  • Document must be issued by Australia State Government Authority
  • Only the front side of the card needs to be uploaded
  • Expiry Date = As displayed on the licence
